DALSA Accessibility Concerns Form

Hello! This form has been created by the Disability Allied Law Student Association (DALSA) as a space for law students, faculty, or staff to report accessibility issues they may encounter on the Lewis & Clark Law School campus.
Once DALSA has received your complaint we will forward it to the appropriate campus departments (e.g., facilities, Dean Davis, library staff, etc.) to address and remedy the issue. All complaints will be anonymized before DALSA takes them to the Law School administration (unless you request otherwise).

While entirely optional, we encourage you to provide your name and contact information so that DALSA can follow up after a solution has been proposed/implemented. We might also need to reach out to clarify the issue if we need more information or guidance.
